Summary of SpayJax Program-This is a short summary of the SpayJax program and how it works. Reading over this will give the you an idea of the mechanics of the program, then after reading this, the answers to most of your questions can be found in the documents and reports listed below.

What Worked, What Didn’t, and What’s Next-This is the good, bad and ugly of the SpayJax program. Some of the ideas were thought were great, turned out not so great, some of the things we thought would never work ended up contributing the most to the programs success.

Marketing of SpayJax-This is an overview of the marketing tactics we have employed to get the word out on the SpayJax program locally. As you will see we were not afraid to try anything, however two types of marketing were the most effective, mass marketing to the entire populace hoping to catch the people we were looking to reach and targeted marketing at citizens receiving government assistance.

Also below you can listen to our radio commercials which were produced pro bono by ClearChannel of Jacksonville. The attached billboards were produced pro bono by the Dalton Agency in Jacksonville and forty of them were displayed for thirty days each by ClearChannel Outdoor for only the cost of the printing.

Veterinary Recruiting Packet-This is the information we include in our Veterinary recruitment package. At least once a year, usually in January, we personally stop by each clinic leaving a recruiting packet and endeavor to see the Veterinarian or decision maker. It seems that we loose and gain a few through the year, but this is the time we most of the clinics are added to our list. We also include in these packets information on our other spay and neuter programs, this information is not included here.

Recruiting Letter to Clinic Owners-This letter introduces our group and goals and specifies how the clinic can become involved. Although this is left at each clinic we really try for a face to face discussion with the decision maker.

Clinic Enrollment Form-This form, as is most of our forms, is simple and straightforward. Nothing fancy and it gets the job done.

Documentation Procedures Booklet-The devil is in the details, and this is where the details can be found. This is the booklet we give to anyone involved in qualifying and documentation of clients for the SpayJax program. This booklet has all the answers to what qualifies clients to participate in the program and what documentation is required to ensure reimbursement.

Client call work sheet-This is the form our hotline workers use to record information about each caller (PDF format).

SpayJax Certificate-This is a copy of the SpayJax Certificate which clients complete with the help of clinic staff to receive pet sterilizations. The top two copies of this certificate are returned to our group along with documentation the bottom copy if kept by the clinic. All of our invoicing to the city and payment to clinics reference these certificate numbers.

Clinic Summary Report Form-This is the form we ask clinics to complete and return with certificates for payment. Notice it requests numbers of sterilization surgeries completed for all clients, not just for our programs. This is to help ensure we are increasing the number of surgeries completed and not just discounting surgeries that would have been completed without assistance. Getting clinics to give us complete information on this form has been difficult, however we will keep working at it due to the importance of these statistics.

Current Marketing Campaign Info-We always have some sort of marketing campaign in the works. This is information about Clinic participation in a recent television promotion campaign.

SpayJax Quarterly and Annual Reports-We helped to write the contract to include the requirement for quarterly and annual reports containing all pertinent information. Attached are a quarterly and an annual report for the first year of the SpayJax program. More recent reports can be downloaded from our website at www.fcnmhp.org

Mayors Task Force Report-The Mayor’s Task Force was a major force in assuring the implementation of the SpayJax program. Attached is a brief introduction and the actual Task Force Report.
